Mosaic Primary Care Network – Sunridge Mall Clinic

Mosaic Primary Care Network (PCN) connects local family doctors, nurse practitioners, and healthcare professionals to support the health of over 400,000 people living in northeast and southeast Calgary.

We work closely with your primary care provider (family doctor or nurse practitioner) to build a personalized support system to help you manage your health and reach your wellness goals. Book an appointment with your primary care provider* to discuss which free programs and services are right for you and your healthcare journey. Through these programs and services, you’ll work with a dedicated team of healthcare professionals, including:

• dietitians
• kinesiologists
• pharmacists
• social workers
• mental health therapists
• nurses

These team members will support you with ongoing support personalized care

*Please note: Some programs and services require a referral from your primary care provider. Find out if your provider is a member through Mosaic’s directory.

ACCESS Clinic 

Mosaic PCN also offers an extended services clinic for evenings and weekends. This is not a walk-in clinic. This clinic only accepts appointments and referrals from Health Link (811).

You can find the entrance on the north-east side of the building opposite Chuck E. Cheese's and beside the Alberta Regional Health Services.
